How can one set up a survey form in Excel?

is there anywhere where there are clear instructions?

Hi Janev, it depends what you want to do?  Do you want spaces provided where
answers can be filled in, or do you want option buttons that can be clicked.  
Do you want to email the skeet to people, or do you want to use hardcopy
forms.  So many possibilities exist, but you have to be more specific, and
provide some specs as to what you want!

Steve,

I have to support you on this one.  Three threads on the same subject within hours in spite of having answers to prior posts.  However, I wonder if the poster is experiencing a problem using the newsreader.

I do my best to maintain etiquette on the forum.  In case I slip and I hope I haven't so far, I ask for forgiveness.

Clarification - The second paragraph is for everyone and all, and *not* just for Steve.  Reason I have to make the distinction is because I have "Steve" as salutation.  

Steve,  I have no regrets so far.  Afterall, maybe we are not that different.  ;)  We are both outspoken.  Having said that, I do realize sometimes "silence is golden."  I should spend my time wisely.
