Good day to all,
I would like to create an on-screen form (for financial assistance
requests), with multiple fields (e.g., date, name, address, phone #, type of
help requested, amount of money pledged, etc.) that I can fill out
on-screen, print, and then have the data dropped into a database. Ideally,
the form would have a "Client #" field that would automatically update each
time the form is opened. I would also like to include either a check box or
radio button that would allow me to indicated if the request is pending or
approved. If approved the dollar amount would be included in the database
to give me a total amount pledged/contributed.
What resources, either books, web sites or whatever, are there that would
give me the nuts and bolts of how to put this together? Or is this
something that is better done in Access?

Glenn, with The Excel Template wizard with data tracking you can create a
template and have it dump into a database (excel, access, or others). You
can use autonumber in Access to create the client number. If you haven't
installed the template wizard you can find it (TMPLWIZ.exe)
