How do I save to csv and get text delimeters?

I'm using Excel 2003 and when I save a file as a CSV it does not ad the
text delimeter. I get

Smith,John A

Instead of "Smith","John A"

How do I get the text delimeters? I've tried setting the format to text
instead of general

Jack,

Normally, the text qualifiers (quote marks) aren't needed unless the field
contains comma or quotes within the data.  But if you need them, get the
Text Write Program from
http://www.smokeylake.com/excel/text_write_program.htm.  There's an option
to put any text qualifier you want around all fields.
