I have created a forms server form and published using the
Administrator Approval process. The process works fine the first
time, and existing forms and new forms can be maintained.
If I use the publish process again to upgrade the form, the classic
"The form has been closed" message displays.
Anyone else see this, and have any fixes?

I'm sorry I don't have a fix, but I have run into similar issues.
Mine was quite strange and a little different than yours. I am
publishing and also used uploading to SharePoint 2007. A month ago my
PC crashed and I reimaged it. Last week I was going to show my boss
how the forms work and got the same error as you. Frustrating and
embarrassing. My only solution - that I could find was to recreate
the forms from scratch.
If there is anyone else out there who knows why this is happening, Bob
and I could use the help.

Bob,
I just fixed mine. Unfortunately, When I went to view the form
through InfoPath, I got a message saying it could not find the
database. Apparently there was something wrong with my ODBC. I had
to recreate that, and fix almost everything on the form including all
of my submit rules.

I have found the best way to debug a web form is to make sure it opens
w/o error as a full client first. Any error in the open process will
cause the web form to display "The form has been closed".
