Query Database from InfoPath Form to auto populate text box fields

I want to create an InfoPath Form that can attach to our client database in
which you can query on a couple of given text box fields to auto populate the
rest of the data field to be able to finish some data entry into the database.

Example:
A new client comes into our facility and a client profile form is filled out
- standard stuff - first/last name, date of birth, ssn, address, city, state
and zip and other demographic information, etc.  During this process when
they are entered into our client master database they are assigned a client
number.

I want to create a form that a therapist can open up to do their Progress
Note in which they can query for the client's first/last name and it autofill
the Progress Note InfoPath form with the client number and maybe a few other
needed fields.

If that is possible, then the therapist would only have to key in the rest
of the clinical billing information.  Then they would submit all of that data
back to a separate database which would contain the client number, first/last
name, diagnosis code, activity code, other clinical billing information, etc.
etc.

I can figure out how to connect the form to the database to query from and
how to connect to a database to submit data to - but not sure how to allow
the form to query the database on one text box field to auto populate the
rest of a few selected but required text box fields for the Progress Note
from the client master database for that client's record.

I know it is long but hope it isn't confusing.

Hey There.

I have a whole system for creating InfoPath clinical notes very closely to
what you describe.

It has database integration, patient lookup or entry. ICD9 codes, etc
They can base the new visit on the previous visit bring forward medication
lists, allergies, diagnoses, etc.

All the data is captured for interface with billing or future use.

Hello,

You would definitely be able to do this with some secondary data sources.  
You could add some buttons to your form that would submit to a secondary data
source and populate text box fields as required, including your progress note
etc.

You would just need to add a "Receive" data source and select the fields you
would like to use as parameters, then set the return data into the fields on
your form.
