How can a form propose a file name when a user is saving a form? Is there a
way to restrict a form so that it saves data only to a given file name? If
so, how is this accomplished?

You should create a Submit operation if you want to control the name that a
user enters for saving a file. InfoPath doesn't have functionality to let
you force a file name as a result of File->Save.
