Customize sample form

I'm using an Applicant Rating sample form to develop a survey form. But I cannot customize the form for example I want to add the criterias to be 10 instead of 5.I cannot add controls to the form and also cannot change the binding of the data source. What should I do?Is there any other ways to customize sample form?

You certainly can add new schema elements into the Applicant Rating sample.
You can't change existing fields in the sample forms, but adding fields
works. What did you try that you couldn't add groups and fields?
