Hi,

This sounds very similar to something you posted not long ago - are you
experiencing the same behavior? If so, you will need to use the same steps
as you did previously. And if you are at the same spot, can you isolate
what you implemented that caused the failure?

As this seems to have occurred for you more than once, it would be good to
try and nail down what you were doing when the form begins to fail.
