Display N/A or Other in Drop-Down from XML file

I am using an XML file as a data source from my cascading drop-down and
would like to include a N/A and other in the list of available
selections.  I used

4e99e84f8b

while building the drop-downs.

Any ideas on how to do this, or will I need to include N/A and Other
for each possible selection?
