I've added a print header and footer to my form (IP 2003). The header and
footer both show correctly in Print Preview. The footer prints correctly on
every page, but the header does not print on any page. I've even tried
pasting my footer text into my header to verify that it isn't anything about
the text.

Is there a setting somewhere that indicates whether to include the header
and/or footer when printing?
