How can I permanently stop the McAfee toolbar from appearing?

I can get rid of it from View and from Tools>customize, but it comes back
with each new boot.

That woul dbe up to McAfee. I would completely remove McAfee's integration
with Outlook. Check the documentation of McAfee on how to do that properly.
