I have a problem with outlook custom form.  I can apply this custom form to
many different folders in the public folders on the exchange server.  However
when I apply this custom form to a specific set of contacts (Already created)
it will not take the custom form format.  I have tried exporting just the
firstname, lastname and importing back into the folder with the applied form
but this has the same result.  Can anyone point me in the right direction.  I
noticed the IPM.NOTE format on the icon for the ones that are correct but the
others use the standard contact icon.

Help me please...

You need to change the MessageClass property of your existing items. Refer
