outlook 2007 dissapears but process keeps running

I have a user who has outlook2007 exchange 2003, his outlook will close
itself , he probably reopens it,  does this a few times
and it eventually crashes.   When he called me in I noticed he had about 5
instances of outlook.exe in process manager.  Outlook was not open on his
desktop however.  I closed all in process manager and started outlook in
safe mode and all of his lost emails came thru.
What could cause this?  We have pretty good AV and malware protection-
router and client end.
Anything I could look for??

Its usually caused by an addin or some app, eg DTS, interfacing with the
data file
