Send/Recieve Accounts Suddenly Disappear; Reappear Upon Reboot

While using Outlook 2003 for a period of time, the four accounts I have
created suddenly disappear in my Send/Receive options.  These accounts
basically have different SMTP server settings and I select the one to send on
based on my current location (office, home, etc.).  I can hit the
Send/Receive button, but Outlook will then start sending based on the first
account in the list, which may not be the account I need to use.

More importantly, upon troubleshooting, I noticed that the Send/Receive
option button, found in Tools, Options, and the Mail Options tab, becomes
inaccessible.

Restarting the computer fixes everything, until the next time they
unexplicably decide to disappear, once again, for no known reason.

Please note that after additional troubleshooting, found that the Outlook.exe
process is not stopping, after Outlook is closed.  Subsequently when Outlook
is repopened, a second Outlook.exe process starts, and this is what is
causing the issue.

So the issue is  really why the Outlook.exe process is not terminating, once
Outlook is closed.

Most likely caused by an addin.
