Outlook 2007 safe mode & Kaspersky anti-spam

Outlook hung and I needed to reboot my computer. When I rebooted, Outlook
started in "safe mode". I looked through help and can't find out how to end
safe mode. Also, the Kaspersky anti-spam toolbar is now missing and there are
no "disabled items" to re-enable. So, the questions are:
1) How do I end safe mode?
2) How do I get Kaspersky's tool bar back?

If you click Help>About, is there a "Disabled Items" button?  If so, what do
you see when you click it?
