HTML forms in Outlook 2007

Hi,
I am trying to insert html code into Outlook 2007 so that I can send out an
email survey (a html form). I have done this successfully with 2003 but with
2007, firstly I can't seem to insert the code into an email so that it looks
like a form and secondly if I send my survey from an email client with 2003
to a client with 2007, the formatting is lost upon arrival, therefore the
form cannot be filled out and no data collected. Could anyone suggest how I
could do this with Outlook 2007?
