Sending messages you don't see are probably stuck read receipts:

Take care of those first and then see what issues may be left behind and

This worked, and cleared up the problem for a couple of days. Then it
reappeared.
I was able to clear it up again, but now I am looking for the underling
problem. What is causing this, and is there something I can do to resolve it?

You should turn off sending read receipts. You should find that in Tools >
Options somewhere. I'm not using Outlook 2003 here at work so I can't tell
you exactly where the option is.
