Need help with FORMS

I am trying to create a form that I can have both
checkboxes and the ability to type on it also. Currently,
if I "protect" the document, I can only select the
checkboxes, I cannot type on it anywhere.

If you need more clarification- let me know!!!

You need to add text form fields, too. See
http://word.mvps.org/FAQs/Customization/FillinTheBlanks.htm and especially
the forms tutorials by Dian Chapman that this article links to.
