I want to map to customized fields in Business Contact Manager

I'm mapping from an Excel spreadsheet into BCM.  I designed a form to include
customized fields, ran and published it.  But then when I start the import
process, the new fields do not appear in the map-to box.

Using BCM v3? Or did you use Outook to add fields to a BCM v2 form?
