Error Message help: "BCM was unable to initiate this form"

Installed BCM update onto a laptop, but when I try to create a new record,
the first tab of the form shows the above message.

Also, none of the BCM toolbars appear - I have checked & BCM is installed
(at least according to the add/remove programs part of the Control Panel)

Hope some of you guys can help out, here!

Go to help > About Microsoft Outlook > Disabled items and enable BCM if
present
