How do I get Business Contact manager setup once the server has ben
configured and online (for 1.5 years)?

We are hosting our own email with the exchange server portion of SBS2003.

Will I have to completely rebuid the server?

Go to the BCM Team Blog
http://blogs.msdn.com/bcm/default.aspx

They've got a Database Tool that will create the db on the server.  You can
use SQL Management Studio to manage it from there and not have to load BCM
on the server.
