I run a business where I need to print out "lead sheets" for my sales reps.
I generate all leads and set all appointmets for my sales reps.  Does anyone
know how to print a lead sheet that includes the customer info & appointment
times and dates.  I would also be nice to list some other custom list like
products disired and notes, but I would be happy with just the contact info
and appointment times.  Things I need are name, address, phone, appointment
times and dates, products the customer is interested in, etc.  Any help would
be appreciated.

I develop a product called Aladdins Office Documents, and I have just
completed the ability to create merge documents in Word from an opportunity.

The Document can contain any info from the Opportunity, the linked Account
or Business Contact and also the Products and Services table can be
recreated in the Document.

I'm not sure if this helps but if not email me and I may be able to work
with you to add other BCM reporting features to Office Documents.
