Contacts Input Screen opens for Busi Contacts?

Had a few Vista & Outlook errors appear the other day and Outlook stopped
working and would not reopen.  MSoft recommended DLoading an Update to Vista.
Did that and still had a few problems, so I reinstalled Outlook w/BCM.  
After a few reboots of my PC I finally got Outlook w/BCM to open properly
again.  Except .....

Now, when I open the "Business Contact" Form for ANY Business Contact all
that opens is the normal Outlook "Contact" Form, even when in the Business
Contact folder.  The "Business Contact" Form seems to have gone AWOL.  
However, under the Addresses Group of the "Contacts" Form there is a note
that says "In Folder <then it shows a card icon> Business Contacts in the
Business Contact Manager".

I guess the good news is the note seems to imply that extra information for
a Business Contact like Source Info, Classification, etc. is still somewhere
............. I just can't see it because only the"Contact" Form is being
retrieved, even when in the Business Contact folder.

Any thoughts how I get the "Business Contact" Form to be once again be
displayed for Business Contacts?

Is this on an existing contact or on a new one created from BCM?

Also, did you backup your BCM database prior to the reinstallation?  Check
C:\Documents and Settings\<yourprofile>\My Documents\My Business\Backups\

Hi Kris ........
It is on existing Business Contacts or even just plain old Outlook Contacts.
The only Input Form that comes up when clicking either type of record is the
standard "Contacts" Form.  And, if I have a Business Contacts View open and I
click Actions/New Business Contact ......... the standard "Contact" Form
opens, not the more detailed "Business Contact" Form.

Yes, BSM database has been backed-up.

Could be Outook forms cache corruption.

Shutdown Outlook.
Delete the .dat file in C:\Users\<user_login>\AppData\Local\Microsoft
\FORMS

And try opening a BCM form again.
