I cannot install my Outlook 2003 Business Contact Manager

I have gone to the program and enabled it.  I have run the installation
wizard and it tells me that installation was successful.  However, the
Business Tools menu never appears.  I have set up at least 7 data bases
trying to reinstall BCM.  The BCM file shows up in the file list on the left
of Outlook but if I try to click on one - such as Accounts - a form comes up
that reads Business contact manager was unable to initialize this form.  It
says on top Untitled- Account and has file, edit , view, insert, tools,
actions, help menus on the form.  Below that it says general, details,
financial history, and finacial summary.

I can't open general.  I can go into the menus but not save anything.  Help!!

Possible problems:

Office and BCM are not on the same Service Pack number. Update the
lesser version.
.NET is hosed. Reinstall .NET.
Forms cache is corrupt. Clear the cache.
Some COM user control is not installed properly. Reinstall BCM.

Thank you for your possible solutions.  I am somewhat at a novice at this.  
I searched updates - critical and optional - and went to the service pack 2
page.  The page tells me I already have service pack 2 and won't let me
re-download it.  I believe my computer - Dell - came with my home office
suite with service pack 2.  I checked my updated and automatic updates
downloaded service pack 3.

I've looked at my add/remove and I have the update for Outlook 2003 with
BCM.  However, I do not see Outlook 2003 with BCM.  Is that included in my
home office suite that came on my computer.  I know it was on my computer
originally - 2005 - because I have an original archived email that says
welcome to BCM.

Next, I will try to reinstall .net but I don't know if I know how to do that.

Is forms cache cleared when I do disk clean-up?  Where do I find forms cache?

Last, I have gone to BCM under Office 8 times and re-enabled it.  I do not
have an office program disk because it came on my computer.

How to clear Outlook's forms cache:

BTW, my comment about keeping Outlook and BCM Service Packs in sync
only applies to 2007 products. With 2003 you should use BCM SP3 and
the latest Office 2003 SP.
