We have had the same thing happen, and I have no idea how it happened or how
to bring it back the way it was. I've tried just about everything in
customize view or defining view. Before, it would insert a row between each
acct when your current view is By Account, and the inserted row would have
the name of the acct and a minus sign beside it so you could collapse it if
you didn't want to see the names in a particular acct. Those contacts that
did not have an acct assigned to them would always be the group on top when
you sorted by acct. It came out of the box with this ability, it was not a
customization on our part. Then one day it was gone, and no apparent way to
bring it back.

A follow up: In the customize view dialog, I am grouping by Account, then by
Full Name, and in Other Settings, clicking Show Items in Group. It then puts
each contact into its own group of one, the account listed by each name is
(none)...which is impossible, most of the contacts in this database have been
assigned to an acct. Opening random contacts confirms they are still assigned
to an acct, its just that the grouping/sorting function of By Account is
ignoring the linked account. Unchecking Show Items in Group in the
customization area changes nothing.
It is also interesting that if you choose to show Account in the column
headers, no acct will be listed that way either. I am beginning to wonder if
there is a corruption.
