Forms for "Accounts" and "Business Contacts" in the "Bus. Cont. Mg

It appears, that the input forms for "Accounts" and "Business Contacts" in
the "Business Contact Manager have dissapeared, after having added four
entries each.
1.) When I wish to add a new entry via File > New > Account / Business
Contact, no form appears.
2.) The Business Contact Manager allows access to the Accounts and Business
Contacts Lists, however when opening one of the entries it is displayed in
the "Contacts" form only and therefor does allow any linking between Accounts
and Business Contacts.

BCM sometimes has what's known as a "forms cache" problem. Clearing the
cache usually fixes it. Choose Tools | Options | Other | Advanced Options |
Custom Forms, then click Manage Forms, then Clear Cache. If simply clearing
the cache doesn't fix the issue, try the other steps detailed at
http://support.microsoft.com/default.aspx?scid=kb;en-us;836558

The bulk of the forms cache issues in Outlook 2003 are fixed in Service Pack
1, so you should also install Office 2003 Service Pack 1 as well as BCM SP1.
