Thanks everyone for such a great forum.  I had the BCM menus dissapear -
common problem and it turned out it was disabled. The solution was right here.

But

I wouldn't have had to spend the last two days installing and reinstalling
outlook, searching the web and otherwise wasting my limited time here on
earth if Microsoft hadn't hidden such an important function as 'disable
add-ins' in the 'help about' menu!   You just countn't get more
counter-intuitive.  

Lets put a link in the options advanced dialog please.

You should direct your suggestion to the Outlook folks.

BCM is in the same boat as all the other add-ins. Add-ins can add
features, but they cannot change Outlook itself.
