I want to schedule a recurring appointment in Outlook that repeats every 4th
day but will also skip sundays.

Set your recurrence to daily, specify every 4th day, and uncheck weekends,
unless you want to include Saturdays, but not Sundays.

Otherwise, you will need to use WS Repeat Appointment available from
http://www.websetters.com or a similar application (none of which I know of
at present.)
