I just realized that I have lost my 2003 and older calendar items.  How do I
make sure that all my calendar, etc. (outlook) items are retained?

I don´t think that you have lost them. They´re just not marked anymore, if
you select a day when you know that you had an appointment, it´s still there.

I checked the days when I did have appointments and I cannot see anything
pre-January 2004- except for recurrence items (birthdays, etc.).  Could this
have happened since my PC synchronizes with my Ipaq PDA? (if actually lost)

I have had several user that lost their calender after sync. I hope you have
your Ipaq with the information, so you can set the sync to owewrite outook
calender items. If you don´t have the Ipaq, the only way to get it back is to
take it from a backup. I hope you use Exchange 2003, so you can use the
Recovery Storage Group
