According to the help function in  Outlook 2000 to Remove a form from a
forms library

On the Tools menu, click Options.

Click the Other tab, and then click Advanced Options.

Click Custom Forms.

Click Manage Forms.

Select the form in either your personal or organizational forms library, and
then click Delete.

Custom Forms does not appear anywhere after clicking the advanced options
button. Can anyone help?

You will need to either change to Corporate/Workgroup mode (see http://www.slipstick.com/outlook/choosingmode.htm) or use an outside tool or code to remove a form; see http://www.outlookcode.com/d/forms.htm#remove
