I don't understand what you mean by calendar "forms" in this instance...do
you mean appointments and events? What view do you have set on the calendar
folder?

I have the same problem.  After doing a search, the Calendar now is displayed
essentially as a spreadsheet.  How do I get it back to a Month/31 view when
that button isn't on the menu and I cannot find how to add it?

Change your view on the Calendar folder back to Day/Week/Month.  You can do
this via View | Current View, or using the Views dropdown on the Advanced
toolbar.
