Is there a way to tell in Outlook on my calendar when a meeting was
originally scheduled?  I.e., If I got into a meeting appointment, is there
any way to tell what day I originally entered the request?

There's a creation date field. You can add it easily to a view by going to a
list view like "Active Appointments", right clicking on the field header
(the line that shows the field names), choose Field Chooser then drag
"Created" from the Frequently-used fields group to the header.
