Outlook appointments on 5th day of month?

I want to set up a recurring appointment in Outlook 2003 that happens only on
the 5th Sunday in a month - months which do not have a 5th Sunday (most of
them!) will not get the appointment. There seems to be no way to do this?

There is no way to do that in plain Outlook.  You need a tool like:
