I can create an appointment and set the time that I want it to notify me, and
it does not alert me at that given time.

Hi John,
if you don´t get a reminder in your default folders, try to start Outlook
with a "Command-line": "/CleanReminders".
Have a look on this site for how to start Outlook with a "Command-line":

Is this appointment in your default calendar? What version of Outlook?
