Can I make an appointment template?

I was wondering if it is possible to make it a template for a new
appointment.  For example, I want to have a table in the notes section so
that every time I click on new appointment, that table is already in the new
appointment.  Is there any way to do this easily without having to make a new
form?  Thanks!

No, but designing a form to have that property is a breeze.

http://www.outlookcode.com has several examples.  Look for the "new default form" instructions.
