how do I cancel an appt but keep archive record of why it was ca

I am a personal trainer and need to keep notes with a canceled appointment.  
Is this possible?  I currently delete appts-, and if they are recurring then
it cancels all appointments in the history.  I want to cancel a single appt,
I know how to do that, but adding a note to a canceled appt and keep in a
archive or file does not seem to be possible.  I would love any input on this
one.

You haven't given us much to go on.  What version? Do you have an
Archive.pst folder?

Let's say you have 2003 or 2007 and you do have an Archive....

You could make a Calendar in the Archive.pst.
Right mouse drag the meeting to that Calendar and when you release say Copy.
Then edit the text to say why it was cancelled.  Maybe even change the
colour of the appointment to show it was cancelled.

Answer those 2 crucial questions and you'll get some more useful and
definitive answers.

You could just assign a Category called, say, "Cancelled" to those
appointments that are cancelled. Remove the reminder from them too.
Alternatively, you could move them to another Calendar folder.
