Not allowing me to create new appointment, form req not displayed

While attempting to create a new appointment in MS Outlook 2003, instead of
the appointment dialog box coming up I receive an error message stating "The
required form could not be displayed, contact your administrator."

Please help.

Clear the forms cache.
