How wrong are they? Have you verified the time zone settings of all of
Windows, Outlook, and the phone?

Yes I have checked the time zone settings on all. The times vary 3-5 hours.

So, it's not always the same offset? Off-hand, I can't think of that might
cause that.

I was wrong earlier. The times are all off by 4 hours earlier on outlook. I
have the right time zone entered in outlook and on phone.

That indicates to me there is a time zone difference, such as between
Eastern time an GMT. Internal to Outlook, times are kept in GMT and Outlook
presents them adjusted for the PC's time zone. If you have sync software
that doesn't take that difference into consideration, you could be seeing
the symptoms you describe.
