Outlook 2003 shared Calender

When the user opens a calender item from another users calender, opens and
attachment within it (while keeping the calender item open), then close the
attachment, than re-open the attachement the following error message than
appears.

"Operation Failed"

The issue has happened on another machine previously when accessing the same
persons calender. It is different calender items as well that this issue
happens with.

What I have done:
1. Detect & repair
2. applied office sp3
3. Renamed user profile and logged her back in creating a new profile
4. re-installed office 2003 and applied office 2003 sp3

Please help

No Changes have been made. They just view the attachment placed in the
calendar item that has been opened and close it. When they attempted to open
the attachment again when the calendar item is still open the error occurs.

When they close the calendar item they opened, than re-open it than they can
access the attachment again.
