Tools, options then right there in the middle of that screen is the default
setting.

I have the same problem and the default setting is set to 15 minutes -- yet
all new reminders are set to 18 hours. Any other ideas?

If you are creating an appointment by double clicking in the calendar, you
are actually creating an Event, which defaults to 18 hours. If you create a
new Appointment using the File->New->Appointment, the default reminder time
will match what you set under the Calendar Options.
