I am looking for code that will enable me to click a link and have it launch
MS Outlook's calendar appointment form filled with information.  Any help is
appreciated.

I'm not sure what you're asking for -- a link or code -- and in what user
context. If you click a link to an iCalendar .ics file, Outlook
automatically opens the file as an appointment.
