I can send appt's to my wife, she accepts and they are added to her calendar.
When she sends an appt to me, I don't get the "accept", only the text of the
appt.  I've checked all our settings in options and they're identical.  Any
help is appreciated.

What version of Outlook are each of you using?  If she is using Outlook 2002
or 2003 and you are on 2000, there is a bug that causes appointment items to
show as text.  Have her turn off reminders in the mail she sends.

Also, ensure that she is using Outlook Rich Text format for sending these
items.
