I can add new fields to views but when I click to type a new contact, I
always get the same form.  How can I add new fields to this form and can I
rename a field that is already on the form?

These pages should help:

   http://www.outlookcode.com/d/forms.htm
   http://www.outlookcode.com/d/contactform.htm
   http://www.outlookcode.com/d/newdefaultform.htm

You cannot rename an existing field.
