This problem could have started in OL2000 and then got transferred
into OL2002. When looking at a report, like the Category List, the
names in replace of email addresses appear in reports in OL2002.

When I go into the individaul contact and click on the emal address, I
get:

The name or distribution list has been deleted and is no longer a
valid address bood entry. The item could not be found.

How can I fix this so that the email address appears and both the
Display Name and email address appear in the send to boxes?

You've provided no information as to how your data became corrupted.
You can try the Inbox Repair Tool, but I suspect your data is damaged beyond
repair.
Your best bet would be to restore your PST file from a backup.
