Deleting obsolete custom forms -- how?

I'm using Outlook 2000 on XP Home.  

With the help of this group, I was able to create a custom Contacts
form to my liking, publish it, set it as default, and convert existing
Contacts.  Hooray!  

In the process, I created several test forms.  When I go to "choose
form," I can see those test forms.  I want to delete them.  How do I
do that?

Since you apparently have Outlook 2000 configured in Internet Mail Only
mode, you'll need to run code to delete any forms. See
http://www.outlookcode.com/d/forms.htm#remove .
