Show contact sub-folders in telephone book

I have been able to locate and add sub-folders from pervious version of
Outlook to the folder list, but can not list them in the telephone book.  I
do have the "Contacts" listed with the names that had been in the old
version.  I also have Personal Address Book and Outlook Address Book, both of
which are empty, listed in the "Show Names From" box.  

How do I add the sub-folders that I created under contacts to the Telephone
Book?

Remove the PAB. You have no need for it. You failed to post your Outlook
version. Read this, and extrapolate to your own version:
