Icon changed for my new form?

Sometime back I created a new Outlook form so I can see the 3 email
address fields all on the same page (I find the default functionality
annoying as it hides Email2 and Email3).

Ever since then, any new contact I create has an icon of a push pin
instead of the usual business card icon

Here's a screenshot of what I am referring to:

Any advice on how to resolve this?

PS This new form is only showing up for new contacts, any ideas on how
I can see *ALL* my contacts in this new form?

Add an icon on the (Properties) page of your form, in design mode, then
republish.
