How do I merge Outlook contact fields not listed by Word

Word 2003 provides a list of available merge fields from an Outlook contact
list.  However, it appears that many fields are not listed as available.  How
do I access these "unavailable" fields?  For example, the "notes" &
"anniversary" fields cannot be merged according the the Word list.

Start the merge in Outlook, not Word, and you'll have all fields available.

What happens if you start the merge from Outlook instead of Word?
