How to share a customized contact form with exchange server users.

I am looking for a way to share a customized contact form with microsoft
exchange users. I haven't found a way that will show the customized form and
information within exchange server outlook. Can this be done? If so, how?

The important issue is -- where is the data that you want to share?

The data is located in my personal contacts folder.

You would need to publish the form to the folder where the data resides and
use that form for all the items in that folder.
