I would like to group contacts into familes so that I can send mail to
households as well as to individuals and so that I can identify individuals
by household. Is this possible using Outlook 2003, and if so, how?

It's a bit of a workaround as Outlook's designed more as a business tool but
try these methods:
you could use the Company field to show the Family surname. Then group by
company.
you could categorise (but you'll have heaps of categories)
